GDPR

Dear Sirs, 

Please be informed that Centrofarm Sp. z o.o., with the registered office at the address: ul. Leszczynowa 1, 05-092 Łomianki, processes information from public and open sources (including from Internet) and from own datasets. Such information may also include details considered as personal data under Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter “General Data Protection Regulation” or “GDPR”)

Considering the above, pursuant to applicable personal data protection legislation, and namely the General Data Protection Regulation, in order to ensure that data subjects are able to exercise their rights, the Data Controller should provide them with information on the processing of their personal data referred to in Articles 13 or 14 of the GDPR. 

In the light of the above, we would like to inform you that:

   • The Data Controller may be contacted by phone, number: 731078444, at the e-mail address office@centrofarm.pl or the correspondence address: ul. Leszczynowa 1, 05-092 Łomianki.
2. The Data Controller processes and may process your personal data for the following purposes:

• marketing – data are processed based on the Data Controller’s need to pursue their legitimate interest (in accordance with Article 6, paragraph 1(f) of the GDPR), through direct marketing of their products and services. In particular, such marketing will be conducted based on your data entered into the customer databases, by sending you solicited information and marketing offers at the provided e-mail address and by presenting them by phone; 
• in case of steps taken before or upon the conclusion or implementation of the agreement for the provision of goods or services for the Data Controller (in accordance with Article 6, paragraph 1(b) of the GDPR); 
• to handle claims, if any, related to provided services (in accordance with Article 6, paragraph 1(b) and (f) of the GDPR);
• to pursue claims, if any (in accordance with Article 6, paragraph 1(b) and (f) of the GDPR).

1. Your personal data may be transferred to third parties to which the Data Controller entrusted the processing of personal data: affiliated companies, providers of IT services, mail and courier service providers, providers of legal services for the Data Controller, self-employed individuals who have a permanent contract of mandate with the Data Controller. 
2. Your personal data will be processed for the following periods: 

• period of existence of the Data Controller’s legitimate interest unless you oppose the data processing for marketing purposes; 
• period required to implement the executed agreement, prescription time for claims thereunder and storage period of documents related thereto (provided that such agreement has been executed). 

1. You have the right to request the Data Controller to grant you access to your personal data, have them corrected, removed, or to limit their processing or their transfer. 
2. To the extent that your personal data are processed for marketing purposes, you have the right to oppose their transfer for such purpose, at any time. 
3. You have the right to file a complaint to the President of the Personal Data Protection Office. 
4. Personal data are provided on a voluntary basis; however, their provision is required for the receipt of marketing materials, and for the conclusion and implementation of the agreement. 
5. The Data Controller will not take any automated decision and will not use the profiling.

• I hereby consent to receiving from Centrofarm Sp. z o.o., address: ul. Leszczynowa 1, 05-092 Łomianki unsolicited commercial information, including marketing offers, via the means of electronic communication, in particular by e-mail, at the e-mail address which I have provided. I am aware that I can revoke my consent at any time. 
• I hereby consent to the use by Centrofarm Sp. z o.o., address: ul. Leszczynowa 1, 05-092 Łomianki of telecommunications terminal equipment and automated calling systems for the purpose of direct marketing, and in particular to the presentation of commercial information on products or services of Centrofarm Sp. z o.o. via such devices as PCs, tablets or Smartphones. I am aware that I can revoke my consent at any time.
